Enrollment Procedure

Welcome to Central Davidson Middle School

 

THE FOLLOWING INFORMATION IS NEEDED TO ENROLL A NEW STUDENT

  • Parent / Legal Guardian must be present and live in our district to enroll a new student.  
    • The registration process MUST BE COMPLETED BY A BIOLOGICAL PARENT or LEGAL GUARDIAN. 
    • This does not include step-parents, grandparents, aunts, uncles, cousins or siblings.
    • An original or certified copy of the student's birth certificate is required by state law 
  • Proof of Residency Documentation (2 forms required) 
    • Current utility bill with parent name and street address
      •  power, phone, water, cable
    • Lease agreement
    • Mortgage statement
    • Tax listing
    • Postal change verification 
  • Immunization Records
  • DRIVER'S LICENSE NOT ACCEPTED AS PROOF OF RESIDENCE but must be presented for notarization of the enrollment form

 

Additional Information Needed

  • All personal information including emergency phone contacts, healthcare providers, etc.
  • Copy of most recent report card
  • Withdrawal form from previous school showing current grades and schedule
  • Most recent standardized test score
    • NC EOG, California Achievement Test, Otis-Lennon, etc.
  • A copy of any special program paperwork where applicable
    • AIG - original placement scores:  aptitude and achievement
    • IEP - Most recent plan
    • 504 Plan - Copy of modifications for local review
